چکیده
Methods A 71 year old male with aortic aneurysm was referred for consideration of surgery. The patient comes to the emergency department to present an acute left chest pain. He was a history of duodenal ulcer and arterial hypertension. Rx thorax demonstrated mediastinal widening compatible with giant aortic aneurysm. The echocardiogram (TTE) confirmed normal sized cardiac cavities and ascending aortic aneurysm. Aortography presented severe dilatation of ascending thoracic aorta with moderated aortic regurgitation. CT Angiography established supra-aortic ascending aneurysm with maximum dimension of 53 mm, 65 mm arch aneurysm and descending 106 mm in the transverse plane. Upper medial resternotomy, cardiopulmonary bypass and circulatory arrest complete with moderated hypothermia were performed as well as ascending aorta and arch replacement with reimplantation of the supraaortic trunks by hybrid prosthetic Evita Open Plus 33.

Hybrid treatment for acute aortic arch dissection.
Acute aortic syndrome affecting the aortic arch remains a treatment challenge, particularly in cases of acute dissection with an intimal tear in this location. Development of hybrid prostheses for complete treatment of the ascending aorta and aortic arch, combined with endovascular treatment of the descending aorta in a single procedure, has greatly increased the possibility of therapeutic succ...

Aorto-left ventricular tunnel (ALVT) is a rare congenital anomaly in which an abnormal communication connects the ascending aorta with the left ventricle, that results volume overload on left ventricle. It is clinically undistinguishable from aortic insufficiency. Its prompt and precise diagnosis is crucial since early surgical treatment is definitely indicated. In this report clinical,...
متن کاملComplications after endovascular repair of Stanford type A (ascending) aortic dissection.
Endovascular treatment has emerged as a less traumatic alternative treatment for several diseases of the thoracic aorta. However, the complications of the endovascular management of ascending aortic dissections are still high. We present a case of two iatrogenic complications after endovascular repair (EVAR) of type A (ascending) aortic dissection. Retrograde aortic dissection at the proximal p...

Objective: In order to determine the optimal surgical strategy for acute ascending aortic dissection, the graft inclusion technique was compared with the open resection technique. Methods: Between 1985 and 1995 a consecutive series of 193 patients (77% male, mean age 58 years) had emergency surgery during a mean interval of 13.2 h after onset of symptoms. Graft replacement of the ascending aort...

OBJECTIVES To describe the early and mid-term clinical and instrumental results of the frozen elephant trunk (FET) procedure using the recent Evita Open Plus hybrid endoprosthesis for elective one-stage treatment of extensive thoracic aortic disease. METHODS We reviewed 16 patients undergoing FET for post-dissection aneurysm (50%), true aneurysm (31%) or other aetiologies (19%), through media...
